Continuum combines clinical file review with a guided, adaptive claimant interview. A physician reviews the resulting information and provides clinically grounded medication recommendations for consideration by the claimant’s treating provider.

Why the file may not be enough

A medication review built around current function.

Traditional medication reviews are limited to the information already contained in the claim file. That information may be incomplete, outdated or insufficient to explain how treatment is affecting the claimant’s current function.

Continuum’s physician-led medication review captures current medication use, recent changes, treatment response, adherence, side effects and occupational impact. The result is a more current, dynamic and functionally relevant medication review—without requiring a comprehensive psychiatric assessment.

Clear clinical boundaries

Recommendations—not direct prescribing.

Technology helps collect and organize current information for physician review; the clinician can verify and challenge it before making recommendations. Direct communication with the treating provider remains central to care.

The program does not prescribe, discontinue or change medication, and it does not replace the claimant’s treating provider. Recommendations are provided for the treating clinician’s consideration.

If the review identifies urgent risk, significant clinical deterioration or unanswered questions requiring specialist assessment, appropriate escalation may be recommended.
