Our story

About Continuum

Continuum Occupational Psychiatry was founded by Dr. David Sarfati with a goal: to make specialist psychiatric care more accessible.

We want to make insurer referrals more feasible by reducing assessment costs and help move disability psychiatry toward physician-led organizations guided by clinical purpose and social good.

About Our Founder

Dr. David Sarfati, MD, CM, FRCPC, founded Continuum Occupational Psychiatry after years of working at the intersection of psychiatry, disability management and functional recovery. He has completed thousands of disability-related psychiatric assessments for a range of reputable organizations, giving him extensive firsthand experience with the clinical questions facing patients, case managers, insurers and treating professionals.

A McGill University-trained psychiatrist and Fellow of the Royal College of Physicians and Surgeons of Canada, Dr. Sarfati is an Assistant Clinical Professor at McMaster University. He is licensed to practise in Ontario, Quebec, Alberta, Saskatchewan and New Brunswick. Through his occupational-health work, he also provides virtual consultations in British Columbia, Nova Scotia, and Newfoundland and Labrador.

His experience extends beyond clinical assessment. Dr. Sarfati has held medical leadership positions in occupational mental health, including serving as Medical Director for the TELUS Health InfluenceCare program. He also developed the medical program for a healthcare startup and served as its Chief Medical Officer. He left that role to establish Continuum around a different vision: a clinician-governed, not-for-profit organization that could combine psychiatric quality, operational responsiveness and more accessible pricing.

Dr. Sarfati has authored and co-authored scientific publications and textbook chapters addressing depression, functional recovery and the effects of mental illness on occupational functioning. He has presented to scientific, clinical and insurance audiences, including at the Canadian Life and Health Insurance Association (CLHIA) Annual Conference.

His professional relationships and experience helped Continuum rapidly assemble a network of reputable psychiatrists, including established leaders in the field.

Continuum is new. The expertise, network and operating experience behind it are not.

Physician-led clinical leadership

Senior Clinical Leadership Council

Meet members of Continuum’s four-person Senior Clinical Leadership Council, which helps shape the organization’s clinical direction, priorities, standards and development.

Founding Senior Clinical Advisor

Dr. Ashwin Ramasubbu

Dr. Ashwin Ramasubbu is a licensed medical doctor and a Fellow of the Royal College of Physicians and Surgeons of Canada. He obtained his undergraduate medical degree at Queen’s University and then completed postgraduate psychiatry residency training at the University of Ottawa.

Dr. Ramasubbu’s passion for occupational psychiatry developed early in his career, when he recognized the significant unmet need in this area. While initially practising as a community psychiatrist, he began completing medical file reviews in disability management, developing proficiency in evaluating functional impairment and providing expert opinions on associated medical limitations and restrictions. To date, he has completed several hundred medical file reviews.

He transitioned from community practice to work as a civilian occupational psychiatrist for the Department of National Defence (DND), where he performed hundreds of general psychiatry and operational trauma assessments to support active Canadian Armed Forces members.

His enthusiasm for occupational psychiatry extends beyond military populations. He continues to assess patients, liaise with case managers and provide clear, comprehensive recommendations to treating clinicians to support functional recovery. He is licensed in his home province of Ontario and holds a virtual licence in Saskatchewan for workplace disability assessments. He also provides virtual consultations in British Columbia and Nova Scotia.

Dr. Ramasubbu values interdisciplinary collaboration and staying current with evidence-based practice. As a Founding Senior Clinical Advisor, he helps shape Continuum’s clinical direction, priorities, standards and development.

Founding Senior Clinical Advisor

Dr. Bryan Chow

Dr. Bryan Chow, MD, FRCPC, is a Founding Senior Clinical Advisor with Continuum Occupational Psychiatry. Through his occupational-health work, he assesses and treats patients across a wide range of psychiatric conditions.

He completed his general psychiatric training and subspecialty training in both addiction medicine and geriatric psychiatry at the University of British Columbia. He is a Clinical Assistant Professor at the University of British Columbia and a Fellow of the Royal College of Physicians and Surgeons of Canada. He is licensed to practise in British Columbia.

He is also a staff psychiatrist with Vancouver Coastal Health, working in consultation-liaison psychiatry and geriatric psychiatry. His clinical work focuses on complex presentations where psychiatric disorders intersect with medical illness and cognitive and functional impairment.

Beyond clinical practice, Dr. Chow is active in medical leadership, digital health and quality improvement at the provincial and national levels. As a Founding Senior Clinical Advisor, he brings broad clinical expertise, strategic vision and a commitment to quality and innovation.

As Director of Psychological Services at Continuum, Dr. Goodman will contribute to the development of psychological services, including psychotherapy pathways, psychological and neuropsychological assessment services, and collaborative approaches to occupational mental health.

The role of insurers

After years working in Canadian healthcare and the disability sector, Continuum’s founder saw the important role insurers can play in connecting people with psychiatric care. An insurer may recognize that a claimant needs specialist support and provide a pathway to assessment and treatment. We see insurers as partners whose work can benefit both the people they serve and the broader healthcare system.

A model built to reinvest

Continuum’s founder chose a not-for-profit model to put that belief into practice. Client fees support the healthcare professionals who deliver services, the administrative teams who make timely access possible, and continued investment in clinical capacity and quality. Surpluses are reinvested in care, research, and innovation rather than distributed to investors or shareholders. Our aim is to keep more of the resources spent on disability psychiatry working within Canadian healthcare.

A network built around clinical voice

Continuum brings together psychiatrists with experience across a range of healthcare settings and established assessment organizations, including for-profit providers. The organization is designed to give physicians a meaningful voice in how services are developed and delivered while remaining responsive to the needs of insurers and claimants.

Continuing to improve

We will continue to study better ways to deliver occupational psychiatric care and use evidence-based technologies where they improve access, efficiency, or clinical quality.

Our aim

By bringing together clinical experience, more accessible pricing, and a commitment to reinvestment, Continuum aims to be a trusted partner for insurers and a stronger pathway to care for the people they refer.
